The Chalkhill Health Centre is moving from the portacabins at Chalkhill and will reopen at its new site, The Welford Centre, on Monday 26 April.
The move, which involves Brent Community Services and two GP practices – the Ellis Practice and the Chalkhill Family Practice – will see both practices closing at 12:30pm on Friday 23 April and re-opening in their new location on the morning of Monday 26 April.

Both GP practices will operate an out-of-hours service from 12:30pm on Friday 23 April until re-opening for emergency only appointments on the morning and evening of Monday 26 April.
The practices will operate as normal from Tuesday 27 April.
